We had an appt with Dr. Shannon (vet) on the 12th of April and she's done lots of research for us as well, we're in contact regularly via text. She suggested trying the Humulin R. I had asked her her thoughts on the Levemir and the mixes. She's not totally convinced the Levemir is stable enough for Bud. So at our appt. he had already eaten and had his insulin about 1/2 hr before. She injected 1 unit of the Humulin R and we tested 1 hr later, he was down to 10.5 (189) beautiful. So our plan with Dr. Shannon's input is ....... up the insulin to 29 units with 1 unit of the R. It seems that this works best for Bud if given at the same time he eats, not after eating. But, I'm sure that can all change too! So then on April 14th, we did this all in the morning (new regime w/the R)........ his fasting BG 2 was 27.l5 (495) and he stayed steady mid to low 20's through the day. Supper he was 20.5 got the same doses (now I know I should have reduced the NPH). At bedtime 9 pm he was 18.4 (331) April 15/11..... fasting is 3.1 (1st) 3.9 (2nd) yep throws Momma a curve that's when I posted this....."So no fast acting this morning. We've fed him and are waiting 1/2 hr to see what his numbers are. Now with the NPH, he was at 29 units, but I think that may need to be reduced in this case....but by how much??? Supper last night I mixed the 1 unit of the Humulin R and the 29 units of the Novolin NPH and he got it at the same time he was eating. Maybe that was the key??? He is also finished his Baytril since Monday (was his last 1/2 tab)" Dr. Shannon replied to text, suggested lowering the NPH to 24 units and no R. He was 11.4 (204) at 7:30 when he got the 24 units of NPH. At 8:00 before I left for work he was 16.5 (297). At lunch when I came to check on him 1:00 pm he was 9.2 (166) ; 5 pm 8.5 (153) and at 6 pm he was 8.2 (148). He got supper and no R but 24 units of the NPH. Checked at 9:3o pm he was 16 (288). April 16/11..... I set my alarm to check him at 2 a.m. 8 hrs after his insulin he was 25.5 His fasting at 6 a.m was 22.1 (397) (so we went with the 1 unit of the R and 29 NPH) mixed with breakfast. At 8:30 he is down to 17.5 (315) Will check him later in the day and update. I guess in hindsight we should have given him 26 or 28, maybe even the 29 at supper last night, then he wouldn't have spiked to the 25.5?? This human insulin is confusing to me But I'm still pleased with the numbers today... If anyone is on line and can comment on the amount, please let me know. Will be checking... Administrator Posts: 7346 04/16/11 01:21 PM Administrator & Owner	Hi Glenda I also used R with Killian... I did include it in the insulin like you did but I didn't like that... I felt less control. There is also a way to include R with N in the same needed.. there needs to be some air between both. I liked using two separate needles better, also I didn't want to just include the R unless I needed to... (and I didn't need to all the time). Killian had high food rises so some R with his insulin meals kept that in check. but I still needed to constantly test... I would never administer R with out testing ..... You will soon learn how the R affects Buddy and will be able to adjust the amount accordingly ... There may be days he don't need it... so dont give it !!! Or in between the shot hours later he may need some.. you can give him some R then... Remember that you wont overlap insulin if you keep in mind that R is usually in and out within 4 hrs.

Posts: 301 04/17/11 08:51 AM	Thanks for the info Johanna. So if I understand you correctly, if he spikes for some strange reason, we can give him the R without any food?? This morning he was 22 so he go his R 1 unit, then had his breakfast and got the 27 units of the NPH. Now I know that he's going to start gaining weight once the regulation is there, but knowing that the R can be given w/o food gives me piece of mind. We can give him a few more calories once in awhile. Like last night, TJ & Tae got bones and Bud got an antler (supposed to be good for teeth, etc.) and I figured less spiking. Well he was NOT impressed; chewed for a bit, then left it. Buddy never just leaves anything. Basically telling us to stick the antler. I gave him one of the bones that still had marrow in it and he was like a little kid. I tested him at 10:30 p.m before bed and he was 22.3 Didn't give him anything else as this was 4 hrs post shot/supper and the NPH should have been kicking in shortly if not already. And I totally agree I would not use the R without testing either. We'd like to slow down on the testing now that his numbers are coming down. However, fasting & meal times will still always be tested. He's been such a good boy about everything, he just deserves a bit of a break. Hair/skin still doing awesome. I think the Omegaderm is working well. very minor itching....and NO PREDNISONE!!! I don't have any experience with R insulin and can't offer any advice but I was just wondering if his bg. spikes weren't due to the antibiotics he was on. Maybe the time will come when he can do ok without the R? He sounds like quite the character..."don't try to slip something different in with me..I know the difference and I want what they're having!" I just don't see how a bone can raise bgs. all that much, but i never had a Rottie who could actually EAT if. My dogs were always small and just licked and mouthed them. and my Lucy, American Pugsley, age 15 -born 4/18/1996, DX. diabetic Nov. 07, blind 2/08, Weight 11 lbs., Humulin N 7 U , Homecooked chicken/white rice with canned Hills W/D My sweet, baby Lucy left me peacefully on October, 24, 2011 with her head on my shoulder and my comforting whispers in her poor little, deaf ear. Vet said to start at 1 unit, seems to be working but at times I'm thinking maybe 1/2 unit more may work better. I'm finding too with Buddy if he gets more protein than normal, spike! Today at lunch he was 12, so I gave him a chicken treat, low fat no preservatives...25 at supper!! But he is doing so much better with the R on board and there are times that he hasn't gotten it due to being lower. Administrator Moderator Posts: 3887 04/18/11 09:26 PM	Glenda I use R for Brandy too. I use 2-3 units with each shot, but I use less NPH then Buddy. I use the R to stop the fast food rise that Brandy will get. Her fasting is between 180-230 so with out the food rise she spends 80% of the day between 100-160. Once in a while if Brandy fasting is below 150 I will cut the R to 1 unit. If she is below 120 1/2 unit now if she happens to be below 100 no R insulin. It took me a while to figure it all out. I tested every hour for the first 5 hours to see how her body was using R. Brandy also spikes from protein I find it take about 6-8 hours for Brandy BG to spike from extra protein. So good to read the R is helping. Dawn

Providing we continue to be vigilant about his treats. Poor Bud....you know he's feeling much better because he's starting to lurk by the treat closet, so we're still keeping the door to the spare room closed!! I will run my thoughts past Dr. Shannon and see if she agrees too. She has been so awesome and thank you for the support here. I feel better about things. So how much NPH is Brandy on? We also need to take Bud to get weighed, hoping he's put on a lb or 2. Administrator Moderator Posts: 3887 04/19/11 06:25 AM	Brandy is on the low end for insulin and she always has been. Now she is getting 14-15 units of NPH along with the regular insulin. She has gained a few needed pounds up to 59 pounds. With her liver issue she started to lose weight. Vet likes me to keep her on the thin side because of the arthritis. I think with all Buddy allergies he might need more insulin. Dawn

Posts: 301 04/19/11 01:09 PM	At 8:00 a.m as I'm heading out the door.... he's 5.2 YAY!!! but then, what is he going to do till lunch because he had the 29 of the NPH. So I gave him a few green beans and 1/2 cu of his food (okay I panicked) Lunch 12:55 he's 14.9 so I guess I panicked too much. Still learning. Also texted Dr. Shannon and she confirmed we can up the R if we wanted but would need to lower the NPH so if he'd have had less NPH today, he's be under 10 now. I am still happy with 14.9 do not get me wrong. I'm so thankful that we've found something that can manage the high BG and thank for the encouragement and knowledge in using it yourselves too. But, I wanted to ask, what is 'Omegaderm' that you mentioned in your post? Cindy and Nash Nash was born 9/2/1998, and is my sweet 1/2 breed Cocker Spaniel/English Springer Spaniel. He was dx'd with diabetes in November 2007 and is currently on 9 units of Novelin N twice a day. Nash went blind approximately 6 mnths after dx, has glaucoma and gets eye drops 5x day

Or do you give it after testing and wait a bit of time before giving the food? Now I know that I need to be careful and if it looks like Buddy isn't feeling well or not wanting to eat....I wouldn't use the R. Next question, if I give him the NPH how long after do you wait? I've tried a couple of different variations and it seems that the R first w/o food, 15 min then feed another 15 min after eating then the NPH. This seems to work well for Buddy. Again I am stressing that if he just wasn't himself and didn't look like he wanted to eat no R..... I can thank my lucky stars that I don't have to deal with this issue for TJ or Tae.....they are no structured eaters by any means. I'd really be having a nervous breakdown. Oh yeah....this morning he threw us a 29!! This way I know for sure what the fasting number is and that she has eaten all the food. This morning she threw me a for a loop over 300 at fasting. Not to panic just gave an extra unit of R to get her down faster. Guess those few extra treats last night did matter. I use the R to cover the food rise Brandy get a fast big food rise. So my vet suggested using the R to stop that raise. I guess I could of played with her diet, but when you have a food a diabetic will eat why fight it. So the R insulin is out before the NPH kick in. Posts: 301 04/20/11 02:15 PM	Just home at lunch, later today, had a dentist appt. at 8:30 before I left he was lower at 23.9 (430) still too high, just check at 13:52 and he's still 20 (360) I think it may be his allergies kicking in, grass finally coming out and he's been out laying in the sun in the back yard every chance he gets, lawn still not done, so it has all the crap from winter....and I'm sure some mold too!! He's been itching his butt because his fur is all wet & curly. Should I give him 1 unit of the R to get him down? I'm home by 4:45 today so that about 2 hrs. Dawn if you're on line, what do you think? With Killian I would test first before the food... I knew he always got a fast food rise, so if he was in the three hundreds I would use about 2 units of R ( I used a separate syringe...) If he was higher then the three hundreds I would go to three units... If he was on the low side I wouldn't use any R at all. Some time I didn't use the R until a few hrs later if he went to high for my liking. So as you see its a matter of knowing your dog and tweaking to fit what is needed .... Posts: 301 04/24/11 11:18 AM	Happy Easter everyone!! Just wanting to update quickly on the Bud.... I'm crossing my fingers & toes that we may have found the right mix here. Yesterday's numbers were awesome (even with his turkey heart treat at supper last night). 06:05 16.8 - 302 10:50 15.3 - 275 16:03 6.6 - 119 18:03 6.1 - 110 10:45 9.7 - 175 I could not be more pleased with a set of numbers like this! When I was brushing him yesterday, I found a bit of a pus filled pocket on his elbow (squeezed it & got myself in the eye....) thinking that may be why he was running a bit higher lately?? put some panalog on it and I'll watch it. This morning his fasting was 16.1 - 290 I LOVE THIS EASTER BUNNY!!! Posts: 301 04/24/11 10:13 PM	Totally intentional Cindy....cuz that is just what he is! Not sure what the elbow thingie was, but I'll watch that area a bit more closely. I know Reiko used to get that too on the calloused part of the elbow. We took my brother back home today, so Buddy came with me again, he's my roadie travelling dude. Trying to get a bit more done at Mom's.... was kind of hard today and now I have a headache and am ready to go home now! Glad the Buddy is with me though. We just came back from a walk around the block, so he's getting settled for the night too. Today supper was 14 - 252 so I'm pretty pleased. I know she is always with me though, and I just try and remember that. Glad that Buddy was a good boy yesterday, and was there for you. Good numbers too, for what, two days in a row! Knock on wood that it continues and it's smooth sailing from here on out. Hope you had a nice day up in your neck of the woods also, the sun was shining here in Michigan and we were able to sit out on the deck and enjoy it!

Brandy and Ruby both get those pimples on their chins. I call it acne and I have been hit in the face too yuck! Brandy been off the fish oil for a while now and hasn't gotten one. Ruby still gets them so I use baby wipes to keep that area clean. I know Buddy takes a supplement with fish oil. Dawn
