The appearance of an eye "shrinking" can have a several possibilities. If it is very noticeable, your vet should be able to say that something is more than "looking great:. However, the eye itself can look great depending upon the reasons. I will explain.

Locations of problems that can have the eye appear to shrink: 1) the eye, 2) the nerves to the eye muscles, 3) the surrounding structures.

1) the eye can shrink. This typically occurs with a perforating event. In this case, the veterinarian should be able to see some changes within the eye. If the eye had other problems, like glaucoma - then the eye can shrink over time. However since both eyes were "originally normal" then this seems unlikely.

2) There is a condition called Horner's Syndrome. It involves a problem with a the nerves to the eye. When the nerve innervation is lost, the eye will appear to sink (called enophthalmos) within the socket and may look like it has shrunk. The affected eye should also have a more constricted pupil (miosis). The third eyelid will often be slightly raised although in a pug this may make it harder to see. The cause can be for unknown reasons (idiopathic), from trauma or other causes.

4) There is also a fat pad and lymphatic tissue behind the eye. Typically these do not shrink for unknown reasons but occasionally problems there can arise.

Finally I would say that the other side should be closely evaluated. If the lymphatic tissue becomes inflamed (such as infection in the mouth) can lead to swelling which will push the other side forward and sometimes the normal eye will appear to have shrunk.

Thank you for the pictures. While it is still very difficult to be sure what is going on, I am encouraged by the fact that I do not see overt signs of abnormal discharge or inflammation. Typically signs that we look for that indicate ocular pain include an eye larger than normal (glaucoma), excess discharge or redness.

I would have the eye rechecked by your veterinarian. A past perforation of the globe that sealed without infection would be high on my list. If this is the case, while vision is impaired greatly, it is often a stable and non-painful situation. Other possibilities exist.

As far as lymphangetasia, while this is a good thought, there would be no connection in this case.
